Please complete the following steps to complete your BCI (FBI Background Screening).

1. Print and fill out your BCI Application. Complete all highlighted sections.

2. Scan and email your application and a current photo ID to info@cryouth.com.

3. The cost of your BCI is $68.25. Please contact 801-602-4039 for payment info. *45 days after your first client placement or DSPD hire date you will be reimbursed for your BCI fee.

4. You'll receive a Live Scan Fingerprinting Authorization Form from Crossroads, please print this form, you'll need this form to schedule your Live Scan.

5. Last, schedule with YUP Finger Printing and find a location near you. You can schedule your appointment online, You will select "Direct Submission/Livescan". 

Please print your Authorization form, bring a copy of your social security card and photo ID to this appointment. Payment for this appointment is NOT required.

Once all of the above steps are completed your BCI application can take anywhere from 2 to 6 weeks to process, Crossroads will notify you when we receive confirmation of approval or denial.
